ACL Injury
ACL Injury
Having an ACL injury is often an overwhelming and challenging time. We understand the physical and psychological challenges that come at every stage.
As a team we are passionate and highly experienced in managing surgical and conservatively managed ACL injuries.
Conservative (Non-Surgical) or Operative (Surgical) management of ACL injuries
Did you know that ACL injuries can be managed conservatively or with an operation. This decision making process is always supplemented with an MRI scan and discussion with an Orthopaedic Consultant.
Factors that will be considered include;
- Partial or complete rupture
- Associated injuries e.g. meniscus tear or ligament tear
- Stability of the knee; is it giving way in usual life or does it feel ok?
- Age
- The sports or activities you like to do
- Future goals
- Timing; you could try conservative first and if it doesn’t work you can move to an operation.
Ultimately, the decision between conservative and operative management should be personalised, based on your goals, lifestyle, and the specifics of the injury.
Prehabilitation before ACL surgery
No matter if you decide to go for non-surgical or surgical management of your ACL it is usually essential to do a period of pre-habilitation i.e. BEFORE any decision is made or before surgery.
The key objectives of this period include:
- Reduction in swelling
- Operating on a very swollen knee can lead to post-operative complications and the potential to develop arthrofibrosis
- Swelling can also inhibit muscle function therefore making the leg feel more unstable.
- Restoration of full extension (straightening of the knee)
- Having a knee which can straighten fully before surgery is
- Improving neuromuscular control in the knee and leg before the surgery
- Improving strength in the quads, hamstrings and calf before surgery results in significantly improved outcomes post operatively.
Rehabilitation following ACL surgery
After your ACL surgery you will usually come to see us on day 5-7 post op. You will be on crutches and will still have dressings on your wounds.
Week 1:
- Check the dressings are clean and dry but won’t change them
- Assess your ability to straighten and bend the knee. Your knee needs to be fully straight by 2 weeks after the operation. Bending the knee fully takes longer.
- Do gait re-education execises with you to ensure you are getting a good walking pattern
- Look at functional tasks such as getting in and out of the chair
Week 2 – 12:
Rehabilitation will then continue approximately weekly. Your Physiotherapist will gradually work on range of movement, strengthening, improving your walking pattern.
Week 12 – month 9:
By approximately week 12 after your ACL operation if you are meeting key objective markers for strength and control you will progress onto hopping and jumping exercises. We will gradually guide you back to running and then if you play a sport we will add in sport specific exercises and drills.
Conservative ACL Management
Your Physiotherapist will work with you to restore range of movement, strength and then once you are meeting objective markers they will introduce you to hopping, jumping, change of direction and sport specific movements.
If at any point you experience instability then we can initiate a conversation with your consultant again and you can reconsider ACL surgery!

What our patients say..
María Safont2024-06-05 Excellent for women's health physio. I wish I had found it sooner and I recommend it to all mum's and mums to be. Alkesh C2024-06-02 Professional experience with a family feel. Very welcoming reception staff. A particular mention to the Jack Collis (physio) who was excellent throughout as well as introducing modern techniques to help with my rehabilitation. Kat Skocikova2024-05-16 I have nothing but praise for Mint Wellbeing. I started seeing Nick every other week for my sciatica, which was ruining my life. Working with Nick was very easy; he shared his knowledge with me, and every week became easier to manage my sciatica. Nick made me feel at ease and, most importantly, he was genuinely interested in my progress and well-being. The entire team at Mint Wellbeing is incredibly friendly and welcoming. The clinic’s interior is calming and clean. Although my physio sessions with Nick have finished, he played a crucial role in my recovery journey and I can't thank him enough. I would certainly recommend this clinic to anyone seeking Physiotherapy. Julie Bishop2024-05-15 Jack is absolutely fantastic. I’ve seen a lot of physio’s over the years and he is one of the best. Honest and practical approach and fast results. My knee feels like it did back in my younger days after treatment from Jack. Thank you Janice Kwok2024-05-08 I started seeing Jenny Fielding in my third trimester for birth prep. I did not attend any other antenatal classes and was very nervous about birth. Jenny is extremely knowledgeable in her field and made me feel so much more at ease. She provided such useful guidance to me and my husband on perineum massage, how to control my pelvic floor and the different relaxation methods during different stages of labour. Couldn't recommend Jenny more highly! Alex Dawson2024-05-01 I have been working with Jack Collis for just over a month with an ongoing knee injury. Jack and the mint team are superb. They are incredibly professional and pragmatic. Even if my insurer was not responsible for settling the fees, I would happily pay privately for their service. Lucy Sapcote2024-04-30 I came to Mint last week for a Post Natal Workshop which was just amazing - a lovely and calming environment that was run by a professional group of women. Loved it so much so that I’ve booked a Mummy MOT off the back of it, as did most of the rest of the group as we were so impressed! Anna Elliott2024-04-23 I first came to Mint Wellbeing for physio with Anita who was so amazing to work with. I've since come back and started attending Pilates classes with Lyndsey and Strength classes with Eleanor which are the highlight of my week. Highly recommend Mint for all their services, the team are amazing! Jess McCarten2024-04-19 Jenny is a miracle worker! I visited half way through my pregnancy with pelvic girdle pain and Jenny treated me in the studio and also gave me some exercises and tips which has pretty much made it disappear! She puts you at complete ease and normalises all pre-natal ailments and concerns making it such a pleasant and relaxed experience! She's clearly highly experienced & educated in this space! I have since returned for a follow-up to discuss birth prep, including guidance on perineum massage and how to do it properly / why we do it! So helpful, cannot recommend enough! George Rawlinson2024-04-11 I have recently completed a series of physio sessions at Mint Wellbeing after a ACJ shoulder injury from skiing. Not only has my recovery been faster than expected, but I’ve received excellent care from my physio Jack Collis.
